Williamston Community Pool & Fitness CTR Liability Waiver (9226)
Memberships
All memberships include use of the Williamston Fitness Center, walking track and lap swim at the Williamston Community Pool. This does not include pool special events or use of the school gyms.  No refunds or prorating will be allowed, but credit may be issued upon approval of the Director. Memberships are in effect for full term noted beginning the date the agreement is signed. You agree to pay for the full amount of the contract you sign for. Access to the track is available during Fitness Center hours only you must check in and have a valid membership, School events take precedence and may cause occasional closures.

Youth 12 and under may not use the Fitness Center. Youth 12 and under must be accompanied by an adult 18+ for any activity unless noted and may utilize the track under direct supervision, this includes Open Swim.

All users of any of the facilities must check in prior to use.

Criminal Check
A Michigan State Police “soft” criminal check must be performed. The check requires us to use the information you provide above in attempting to identify those individuals that may pose a physical threat to children and adults at the school. This information will remain confidential.

NSF checks or declined credit card payments will have a $35.00 surcharge. There will be a $10.00 late fee for all late payments.

**If Williamston Community Schools are closed due to inclement weather, the Williamston Fitness Center and Pool will open at 9:00 am for morning hours.

Assumption of Risk
Participation in Williamston Enrichment, Pool, and Fitness Center - programs and/or use of its facilities is completely voluntary. Participation may result in physical and/or mental injury from these activities. Individuals participate AT THEIR OWN RISK and assume responsibility for their own health and safety. Williamston Enrichment, Pool, and Fitness Center is not liable for injuries sustained during participation in sponsored activities or as a result of using its facilities.

It is strongly recommended that all participants consult a physician and/or have a physical exam prior to participation. Participants should understand that Williamston Community Schools does not provide personal accident/health insurance. Therefore, participants are urged to secure their own insurance.
